Practice Review options

A. Full assessment and report (recommended)

This 3 day consultancy will provide your practice with a comprehensive review of practice systems and processes, culminating in a report and recommendations. As part of the process we provide our innovative software product (Practice Focus) to support management of chronic diseases. The format is generally as follows, but can be adapted to suit your requirements.

Pre visit – obtaining baseline data

Our consultant will ask your practice to complete a questionnaire to help us understand your practice systems and processes.

With your permission we will run a suite of clinical audits using a remote log in facility. This will enable us to identify potential opportunities for your practice.

Day 1 - First visit

One day visit from QMS consultant to obtain baseline data using Practice Focus.

Interview with the practice manager to clarify organizational issues and practice systems and processes, in particular those of relevance to QOF and chronic disease management (CDM).

Day 2

Draft of report, highlighting;

• risk areas
• opportunities for quality improvement
• opportunities for income generation
• data quality and register issues, maximizing prevalence
• opportunities to enhance practice organization, systems and processes, including recall systems and IT

Using the data obtained from our meeting together on day one, we strive to make the report relevant to your practice culture.

We also provide supporting documentation including Read code aide memoirs, exception coding analysis, and systems for recall. 

Day 3 – Second visit

Return to the practice.

• Presentation of draft report.
• Installation of our Practice Focus software to support systems for chronic disease management
• Training to gain maximum benefit from the Practice Focus .

We know you are keen to ensure your practice achieves its targets, and maximizes income, without burdening clinicians, and we are confident this report will help you to meet these objectives.

If we cannot make recommendations which would result in increased income for the practice, we will refund the cost of the report!

1. How can QMS increase my practice income?

The QMS team will run multiple searches on your clinical system to ensure your disease registers are accurate. We will demonstrate potential additional income available to your practice, if you improve disease registers and maximize prevalence. We will review performance against all QOF targets, including exception coding, and identify priority areas for your practice team to focus on. 

2. How can QMS help to set up recall and review systems?

QMS has developed a systematic process to ensure chronic disease management (CDM) and recall systems are efficient. This will be adapted to suit your practice, and run electronically. It includes mail merge options to ensure efficient and effective invitation of patients for recall. We will demonstrate systems for tracking invitation letters and updating exception coding. 

3. Can QMS help make my practice systems and processes more efficient?

QMS latest software products can be used alongside your current practice clinical system. The software generates electronic plans to support the practice in achieving performance targets and ensuring high quality of patient care. 

4. Can QMS help me protect clinicians from burnout?

The QMS team will demonstrate opportunities to enhance the role of your admin team to support chronic disease management (CDM), removing burden from your clinicians.
